Please note: There is a flat travel fee.

Steps to booking a reservation through Survive & Thrive:

Step 1. Attain a Doctors Note from your Oncologist explicitly stating what services you can have performed on you from the list above. This gives us permission to perform the service.

Step 2. Email us the doctor’s note along with your name, address, & desired service.

Step 3. We will call/text/email you to give you available dates/times for your reservation.

PLEASE NOTE THE DOCTOR’S NOTE MUST BE RENEWED EVERY 6 MOS.
